I have a few questions about email query format:

Do you put the agent's address on the top of the email as with the snail mail? How about the date your send your email? Do you leave blanks lines between "Sincerely" and your name at the end of the email?

I participated in a query roulette with an agent and this is what she said to one person who put the address at the top:  In an electronic query letter don't put your address and the agent's address at the top of the page. That's for paper queries only.

I wouldn't date it.  Email has a date and time when it's sent.

I just say,

Thank you,
Name
Contact Info

No blanks

Hmmm.  I think there's no hard and fast rule on this one.  Personally, I prefer a formal looking email, because it looks more professional.  But that's just me.

There aren't many standards for this yet but putting things like names and addresses at the top of an email looks odd to my eye so I start with "Dear Wonderful Agent's Name:" and put my name and address below my signature. That is done so frequently that it isn't likely to raise any eyebrows. Of course, any letter should be single spaced and with a double space between paragraphs whether it's snail or email.
