I am interested in a particular agent that has an online form. On the form there is a box that says, Pitch/Summary/Synopsis. Can anyone clarify what this box is asking? Is it actually asking for three different things or am I suppose to chose?

depends on how many characters you are allowed.

I would assume it's all three if the number is very high.

I would maybe check on their querytracker page and see if you can get clarification from others who have queried them.

I'd assume they mean "the middle part of the query that isn't the bio and word count part," particularly if the form has different boxes for word count and bio.

Sometimes people call the middle part of a query the synopsis.
Sometimes people call it the pitch.
Sometimes people call it the summary.

The agent listed all three so no matter what you call it by, you know what to past there. Which, as gckatz said, is the middle of the query where it tells about the plot of your novel
