Couldn't find specific guidelines or a website or anything, but judging by comments below here, I just sent in a query letter. Seems like if she wants sample pages, she'll request them.

First, I emailed the address posted here on querytracker to make sure it was appropriate to send queries to this address.
She replied in less than 24 hours.

EQ: 7/3/2019. Let's see how this goes. She reps one of my favorite books, so I know better than to get my hopes up too high.
